• The initial visit is a comprehensive care planning visit (typically more than one hour) which is included in your direct primary care membership ($500 value). This visit is typically a telemedicine visit or home visit. 
• All subsequent visits will be fee-for-service, and as a direct primary care member, you will enjoy special courtesy cost reductions for these visits. 
• Rapid and secure communication with Dr. Boxer via telephone, email, or portal messaging. 
• You will communicate with Dr. Boxer and Dr. Boxer alone! No front-desk staff, medical assistants, billers, nurses, NP’s or PA’s, and no phone menus!
• Secure Patient Portal with 24/7 access to your medical records. 
• Be seen in the office, in your home, or by telemedicine.
• Emergency visits on evenings, weekends, or holidays if needed. 
• Monthly email “check-in’s” with Dr. Boxer. 
• Monthly medical blogs on topics of your choice. 
• Detailed invoice will be provided for submission to your insurance company for reimbursement for fee-for-service visits.
